General Guidelines on Forms

There are a variety of forms – such as Originating Application for Divorce, Originating Application for Nullity, Originating Application for Extension of Time – that can be used for the filing of applications to the Family Justice Courts.

To understand when each form should be used, along with the relevant Rules and/or Practice Directions (“PD”) paragraphs that apply, you can refer to the Table of Forms.

If there is no specific court form that can be used for your application, you can use the general forms instead:

  • Generic Originating Application (Form 53A)
  • Generic Summons (Form 67A)
  • Generic Affidavit (Form 54)

Forms are organized according to the sequence of rules, which follows the general life cycle of a case.
